Excludes Accident ONLY cover, Ambulance ONLY cover, Extras ONLY cover, Overseas Visitors Cover (OVC), Overseas students health cover (OSHC), Working Visa Health Insurance, Travel Insurance , Pet Insurance, Funeral Insurance, Life Insurance, ahm cover and all corporate covers. AMCAL gift card terms: Must hold the same eligible cover and not be in arrears for 65 consecutive days from the policy start date. Family and couple policy holders will receive a $400 Amcal gift card, and single and single parent policy holders will receive a $200 Amcal gift card. The gift card will take up to 12 weeks to arrive by post after the policy start date. The gift card is only redeemable at Amcal Pharmacies. The gift card balance expires within 12 months of the issue date. The gift card is not refundable and cannot be redeemed for cash. Lost or stolen cards will not be replaced or refunded and no liability will be accepted for the loss, theft or destruction of gift cards. 3 Live Better points will be displayed in your Medibank Live Better account within 3 days of making an instore purchase. Live Better points will not be earned on purchases from Amcal online, prescription purchases or purchase amounts that have been paid for with an Amcal e-Gift Card, Gift Card or coupon, or via Amcal Rewards points. Live Better points will not be applied unless the Medibank Live Better account is successfully linked to the Amcal Rewards membership account prior to purchase. Each Medibank Live Better account can only be linked to one Amcal account. Live Better points cannot be awarded retrospectively. This offer may be withdrawn or change at any time.
4 Amcal members will earn Amcal Rewards points on all purchases they make at any participating Amcal pharmacy or at www.amcal.com.au, other than for excluded items. Excluded items include gift cards, prescriptions products advertised as being excluded and products where the granting of points would be otherwise be prohibited by law. Amcal Rewards points will not be earned on redemptions. Members will receive 1 Point for every 20¢ spent (5 Points per $1.00) on non-excluded items. Bonus points may be offered from time to time. If you do not have your Amcal Rewards Card with you at the time of purchase you can return to the point of purchase within 30 days with a copy of your receipt and your Amcal Rewards Card and the points will be credited to your account. Transactions processed online must include your Amcal Rewards membership number in order to earn points, points cannot be allocated retrospectively to online transactions. Secondary Amcal Rewards Cards will not earn points for online transactions.

5 Eligible members on Medibank extras (excluding Healthy Living Extras and Gold Ultra Health) can claim a maximum of two 100% back dental check-ups per member, per year at a Members’ Choice Advantage provider (including bitewing x-rays where clinically required). For members on eligible extras, the first two check-ups do not count towards your annual limit. Members with Healthy Living Extras can get 100% back on one dental check-up each year at a Members’ Choice Advantage provider (including up to two bitewing x-rays, where clinically required) or at a Members’ Choice provider (excluding x-rays). Members with Gold Ultra Health can get 100% back on up to three dental check-ups at a Members’ Choice or Members’ Choice Advantage provider. Members’ Choice and Members’ Choice Advantage providers are not available in all areas. Some products may have other dental benefits, check your cover summary for details.
6 Excludes Healthy Living Extras and select products that are no longer available for sale (for more information check your cover summary or check this page). For ambulance attendance or transportation to a hospital where immediate professional attention is required and your medical condition is such that your couldn't be transported any other way. Tasmania and Queensland have State schemes to cover ambulance services for residents of this States.
11 Waiting periods you have not fully served with your previous fund may still need to be served. Waiting periods for any new or upgraded services or higher benefits apply. Benefits that have been paid under your previous cover will be taken into account in determining the benefits payable under your Medibank cover.
