Optical Dispenser

For recognition with Medibank as an Optical Dispenser you must have an Australian Business Number (ABN).

Send the completed application form and certified true copies of the appropriate documentation to Medibank either via: 

• Email: ancillaryregistrations@medibank.com.au
• Mail to: Ancillary Provider Registrations
               Medibank Private Limited
               GPO Box 9999 (your capital city)

As an ancillary health care provider, Medibank expect providers to meet the recognition criteria and standards as listed below:

• Recognition Criteria for other ancillary health care providers
• Ancillary Billing Standards
• Patient Record Standards
• Optical Standards
