How to claim for dental services
Making a claim for your dental treatment is simple when you understand how. On this page you can find out how to claim for your dental surgery, and general or major dental~.
Note: when reading, keep in mind we use the word "dentist" to describe dentists, periodontists and orthodontists.
How to claim for dental services
If you’re being admitted into hospital as an inpatient for your dental surgery and have hospital cover that includes dental surgery, your claim has two parts:
a) Claiming for related medical services, such as the anaesthetist, with Medicare before claiming from Medibank. You can identify these by their 5-digit item numbers.
b) Claiming for the dental portion of the surgery through your extras. You can identify this by its 3-digit item number.
To claim for medical portion of your surgery
You can claim from both Medibank and Medicare by either:
- Lodging a claim with Medicare using the Medicare two-way claim form. This is the quickest and easiest way to claim from both Medicare and Medibank. You can opt to pay the bill in full and lodge as a ‘paid claim’, or not pay and lodge as an ‘unpaid claim’. When you submit to Medicare, you'll also have to include a completed Medibank Claim Form – select "Medical" as the claim type.
- Lodging a claim with Medicare with the Medicare Claim Form and then lodging a claim with Medibank, providing the Medicare Statement of Benefit.
To claim for the dental portion of your surgery
This is an extras claim. You can do this in any one of two ways:
- On the spot with your Medibank membership card or digital card if they offer HICAPS or ISOFT.
- My Medibank claim online or through the app

If you haven’t paid for the service or product yet, you can only make an offline claim. Please download and complete this Claim Form. You will need to include the original invoice with your claim. Post your completed claim form and documents to: Medibank, Medical and Extras Claims, Reply Paid 2984, GPO BOX 2984, Melbourne VIC 3001.
Alternatively, submit your completed claim form and documents in person at a Medibank store.
You can claim for someone else on your policy, but you can’t submit a single claim form for more than one person at a time. You’ll need to do a separate claim form for each person. Please keep in mind that unless we have the other person’s consent, we won’t be able to discuss the details of the claim once it has been submitted.

Our handy Out of pocket estimator will give you a good idea of what your total costs will be, so you know how much you’ll get back when you claim. It’s simple to use – just tap on “Out of pocket estimator” on the main menu of the My Medibank app and follow the prompts.
It may seem surprising, but like all tooth extractions, having your wisdom teeth out is usually included under general dental in your extras. However, when you go to hospital there are other fees apart from the extraction itself, like hospital theatre fees, anaesthetist charges, and accommodation if necessary, and you’ll want to make sure you can claim for these.
