How to claim for a Hospital treatment or service

You have an itemised receipt or invoice dated within the last two years. This should show the provider name and address, the item description for your treatment/service, the total cost for the treatment/service, and if you've paid for it.

For Australian residents: if you were a private hospital inpatient, you'll need to provide a National Private Patient Hospital Claim Form (HC21) signed by the hospital and patient. If you were a public hospital inpatient, you'll need to provide an election form.

For overseas health cover: you'll need to provide a National Private Patient Hospital Claim Form (HC21) signed by the hospital and patient.

For CPAP (or similar) devices: you'll need to provide proof of completion of an overnight sleep study or sleep study results. Note: if this was an inpatient service and has already been claimed with Medibank this is not required. If you're claiming for a replacement CPAP or similar device and have already claimed with Medibank then you don't have to upload any additional documents. If you've hired or purchased a CPAP device you'll need to upload a receipt. You can only claim this if the hire or purchase was within 12 months following the sleep study.

For cochlear implants and sound processors: you'll need to upload your application form. This would've been provided to you by the audiologist.
